Unfortunately do to insurance restrictions, we are unable to see any work related or automobile accident problems. Your medical insurance will deny all claims and you may be left personally responsible for any bills.
Dr. Miller is an Adult Neurologist treating patients over the age of 18. In special circumstances we will see a younger patient, but please call the office to discuss at 410-760-5599
We ask new patients to complete 4 forms prior to your visit: Patient registration, Medical questionnaire, Financial agreement, and Consent form. All of these forms can be completed on our website under Patient Forms. Alternatively, you may download, print, and mail the forms along with a copy of your insurance cards and photo ID to our office.
A referral from your primary physician is essential and required in order to be seen, as this is a requirement of all insurance providers.
If you have previous records or test results that you would like your physician to review, we encourage you to bring a copy of those records with you to your appointment.
We value your time and try very hard to avoid long waits at our office. One late patient can cause everyone else to be be delayed so we kindly ask you to be prompt for your appointment. We also try very hard to schedule patients who have urgent needs, so a missed appointment is wasted time that could have been filled by someone who has been waiting. For this reason, we have instituted a $25 fee for missed appointments.
If you need to cancel or reschedule your appointment, please call us at least 24 hours before your scheduled appointment time. If the office is closed please leave a detailed message with the patient’s name, DOB, and a call back number.
If you have a last-minute cancellation (within a 24 hour window of your appointment time), please call the front desk as soon as possible and we can reschedule you.
If you arrive more than 15 minutes late for your scheduled appointment, it will be considered as a no-show and you may need to be rescheduled.
New patients who do not show nor call to cancel their first visit will NOT be rescheduled and exceptions are made on a case-by-case basis by the discretion of your physician.
Please note that existing patients with excessive last minute cancellations or no-shows may be discharged from the practice at the discretion of their physician.
An EMG requires recording nerve electrical potentials through the skin. Small or damaged nerves are very difficult to accurately record. Any lotion on the skin will impair our ability to reliably record these potentials. We ask that NO LOTION of any kind be applied to the hands, arms, legs, or feet for 48 hours prior to the study. This includes hand sanitizer that often has lotion included. Deodorant is allowed.
We accept most commercial insurances (BCBS, Aetna, United Healthcare, Cigna, etc.), Medicare, and Medicaid. We do not accept Maryland Physician's Care and JAI Medical.
Patients are responsible for obtaining any necessary referrals for health plans that require a referral from a primary care physician. If you do not have a valid referral for your visit and you require one you may be charged out of pocket for your visit or testing.
